Branch Lahore Ac no= 3164498127 or under the heading CE UHS before applying online.
Applicants must enter Bank Challan in the online application form.

Candidates must print the online form if a download option is available.
Submit your application with notarized copies of the following documents: It must be submitted to the secondary registrar (Admin & Coord, University of Health Science, Khayaban – e – Jamia, Punjab, Lahore).
